Registration
List of registered schools
Schools may register in any of the following ways:
- Complete the registration form at http://zimmer.csufresno.edu/~mnogin/registr-form.html.
- Send an email to the Field Day Coordinator, Maria Nogin (mnogin@csufresno.edu) with the following
information:
- Name of school
- School's enrollment
- Adviser's name
- Email
- Mailing address
- Phone number
- Approximate number of participants
- Mad Hatter A (grades 11-12)
- Mad Hatter B (grades 9-10)
- Leap Frog (any grades, teams of 2 students)
- Game (any grades)
- If you have received a registration form by ground mail, complete it and return to
Dr. Maria Nogin

(*) The registration fee is $1.00 per student. You can pay by check (payable to CSU, Fresno Foundation -
Mathematics) or cash, in advance or at the registration desk on the day of the event.
If your school does not have (or if you are not sure whether it has) a Math Field Day team, individual participants may register in any of the following ways:

- Send an email to the Field Day Coordinator, Maria Nogin (mnogin@csufresno.edu) with the following
information:
- Participant's name
- Grade
- School
- School's enrollment (if known)
- Email
- Mailing address
- Phone number
- What contest you wish to participate in:
- Mad Hatter A (grades 11-12)
- Mad Hatter B (grades 9-10)
- Leap Frog (any grades, teams of two students) (**)
- Game (any grades)

(*) The registration fee is $1.00 per student. You can pay by check (payable to CSU, Fresno Foundation -
Mathematics) or cash, in advance or at the registration desk on the day of the event.
(**) If your school does not have a Math Field Day team but you and your friend(s) (from the same school) wish
to participate, one of you or one of your parents (or anybody else) may act as your adviser. In this case, use
school's registration (see above).
Deadlines
- If you wish to apply for our registration fee waiver, please do so by January 1, 2005 to ensure
full consideration. We do not need to know the exact number of participants at this time, although a rough estimate
would be helpful. You will receive an answer by January 10. Priority will be given to the first time participants.
- To ensure that the name of your school appears on the list of participating schools in the booklet,
please register by March 15, 2005. We would like to know the number of participants for each contest by that time so
that we know how many copies of the materials to print.
